Varus Posteromedial Instability of the elbow results most typically following a fall. Its hallmark pathologic features include disruption of the lateral collateral ligament (LCL) and a fracture of the anteromedial facet of the coronoid process. While evaluation with plain xrays may be adequate to diagnose this type of instability, a CT scan may at times be helpful in establishing the size and exact configuration of the coronoid fracture. An accurate understanding is important since failure to provide operative stabilization of the fracture for this type of elbow instability can result in chronic instability and a poor functional outcome.
Treatment requires LCL repair and open reduction and internal fixation of the coronoid fracture. This typically involves a butress plate. Satisfactory outcome requires both an intact LCL and healed anteromedial facet of the coronoid process as both resist varus stress.
